29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  On or about November 23, 2022, and at times prior thereto, inspections of appropriate frequency were not conducted where roofing activities were being performed by workers, exposing them to hazards such as, but not limited to fall and struck by hazards.

29 CFR  1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  On or about November 23, 2022, and at times prior thereto, five workers performing work on the roof of a multifamily home under construction gained access to the roof of the building by climbing up and over the guard rails of an Aerial Platform without fall protection, exposing employees to a fall hazard.

On or about November 23, 2022, and at times prior thereto, an employee was performing work on the roof of a multifamily home under construction, 6 feet or more above the lower level without the use of fall protection, exposing the employee to a fall hazard.

29 CFR  1926.502(d)(6)(ii):Snaphooks not designed to be connected to one another were engaged to each other.  On or about November 23, 2022, and at times prior thereto, five workers performing work on the roof of a multifamily home under construction were using two Snaphooks wrapped around a job made fall protection anchor and connected to each other without being designed for that purpose, exposing employees to a fall hazard.

On or about November 23, 2022, and at times prior thereto, five workers performing work on the roof of a multifamily home under construction did so without a fall protection anchor capable of supporting at least 5,000 pounds per employee attached, exposing employees to a fall hazard.
