29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas On or about November 29, 2022, and at times prior thereto, debris with protruding nails were present in and around the multifamily home under construction where roofing activities were being performed, exposing workers to hazards such as, but not limited to puncture wounds, and lacerations.

29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about November 29, 2022, and at times prior thereto, five workers were using pneumatic nail guns to frame a multifamily home under construction without using eye protection, exposing the workers to the hazard of being struck-by flying objects.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about November 29, 2022, and at times prior thereto, five workers were performing work on the roof of a multifamily home under construction, 6 feet or more above the lower level without the use of fall protection, exposing the workers to a fall hazard.
