29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. a. White River Safety Inc., Wilson Creek Unit 69, Rig #5545 2500 County Road 9 Meeker, CO 81641: On June 29, 2022 the employer did not ensure that the rig floor of Ranger Energy Services Rig #5545 was kept in a clean and sanitary condition. The employer was observing a stripping job being performed on a well that contained a continuous rod. To perform this stripping job, the rig crew was pulling wet strings of tubing from the well without implementing a way of conveying the fluids away from the rig floor. The rig floor was covered in wax paraffin, oil, and flammable liquids. This condition exposed employees to slip, trip, and fall hazards.

29 CFR 1910.252(a)(1)(iii) Welding or cutting was performed, when the requirements stated in 29 CFR 1910.252(a)(1)(i) and 29 CFR 1910.252(a)(1)(ii) could not be followed: a. White River Safety Inc., Wilson Creek Unit 69, Rig #5545 2500 County Road 9 Meeker, CO 81641: On June 29, 2022, the employer did not ensure that requirements were met prior to cutting a continuous rod over an open well. All fire hazards were incapable of being removed to a safe location and guards could not be used to protect the immovable fire hazards. These requirements must be followed to allow cutting operations to be performed. This condition exposed employees to fire and explosion hazards.

White River Safety Inc., Wilson Creek Unit 69, Rig #5545 2500 County Road 9 Meeker, CO 81641: On June 29, 2022, the employer did not ensure that a Milwaukee 6230 Bandsaw was approved for the Class 1 Division 1 location it was being operated in. The non-intrinsically safe bandsaw was used on the rig floor of Ranger Energy Services Rig #5545 to cut continuous rod over an open well. The crew was pulling wet strings of tubing in the same location the non-intrinsically safe bandsaw was being used. This condition exposed employees to fire and explosion hazards.
