29 CFR 1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about November 30, 2022, a flexible cord was connected to a Square D 112.5 KVA electrical transformer and did not have strain relief. This condition exposed employees to electrical shock hazards.
