29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  (a) On or before December 5, 2022, the employer did not ensure that each employee working around an excavation at ground level was protected from injury in that an employee carrying turnbuckles tripped and fell into the excavation.  Tarps, various pieces of material, clumps of soil, and un-even ground were located near and at the edge of the excavation.  This condition exposed employees to trip and fall hazards of approximately 10 feet.
