29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse. a)Jobsite: Warehouse located at 2900 Titan Row, suite 100, Orlando, FL: pallets with vinyl boxes were improperly stacked 3 tiers high reaching heights of up to 18- feet 6-inches, in that they were not securely stacked, blocked or interlocked, exposing employees to crushing hazards, on or about December 3, 2022. b) Jobsite: Warehouse located at 2900 Titan Row, suite 100, Orlando, FL: pallets with vinyl boxes stacked 2 tiers high reaching heights of up to 12- feet 6-inches, were observed leaning exposing employees to crushing hazards, on or about December 6, 2022.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about December 3, 2022, at 2900 Titan Row, suite 100 warehouse, the employer failed to report a work-related in patient hospitalization of an employee to OSHA within twenty-four (24) hours. The media notified OSHA on December 5, 2022, of the in-patient hospitalization.
