29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  (a): On or about August 17, 2022, the employer did not ensure that all areas where hazards were created by point of operation, ingoing nip-point, and rotating parts were guarded on machinery, such as the Line 23 GSD650FSL-M Quad-Seal Flat Bottom Pouch Making Machine.  This exposed employees to caught-in hazards and amputation injuries.  (b): On or about December 6, 2022, the employer did not ensure that all areas where hazards were created by point of operation, ingoing nip-point, and rotating parts were guarded on machinery, such as the Line 22 GSD650FSL-M Quad-Seal Flat Bottom Pouch Making Machine.  This exposed employees to caught-in hazards and amputation injuries.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  (a): On or about August 17, 2022, the employer did not provide the required injury or illness description on the OSHA 300 Log for the calendar year 2022. The employer had 5 recordable injuries listed on the OSHA 300 Log, and the description in Column F lacked the required information, including, (but not limited to) the object or task that caused the injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  (a): On or about August 17, 2022, The employer did not report an employee's amputation. The employee was operating the GSD650FSL-M Quad-Seal Flat Bottom Pouch Making Machine when her finger was amputated by in ingoing nip-point. The employer was aware the injury occurred and did not alert OSHA of the incident.
