29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) On or about October 26, 2022, at 96202 Queen Palm Way, Fernandina Beach, Florida: the employer failed to report a work-related in-patient hospitalization of an employee to OSHA within twenty-four (24) hours. The employer notified OSHA on December 15, 2022, of the in-patient hospitalization.
