29 CFR  1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a. Located at Fishinger Road and Sciotangy Drive in Upper Arlington, Ohio: On or about December 5, 2022, employees were exposed to cave in hazards while performing pipe laying work from a trench excavated in the roadway. The employer did not ensure that the hydraulic shoring was installed in accordance with the GME tabulated data. Two consecutive shores were not in place to provide a safe working area between the shores.
