29 CFR 1928.57(b)(1)(ii):All tractors were not equipped with an agricultural tractor master shield on the rear power take-off except where removal of the tractor master shield is permitted by paragraph (b)(1)(iii) of this section. The master shield shall have sufficient strength to prevent permanent deformation of the shield when a 250 pound operator mounts or dismounts the tractor using the shield as a step. (a) During feed milling -- On or prior to 12/04/22, the John Deere 5425 tractor was not equipped with a master shield covering the PTO drive , exposing employees to caught-in hazards.

29 CFR 1928.57(b)(1)(iii): Power take-off driven equipment was not guarded to protect against employee contact with positively driven rotating members of the power drive system. Where power take-off driven equipment is of a design requiring removal of the tractor master shield, the equipment shall also include protection from that portion of the tractor power take-off shaft which protrudes from the tractor. (a) During feed milling: On or prior to 12/4/22, the PTO shaft for the International Harvester 1150 Feed Mill was not guarded, exposing employees to caught-in hazards during operation.

(a) During feed milling: On or prior to 12/04/22, the flywheel, drive belts, and chain drive assembly(s) for the International Harvester 1150 Feed Mill were not guarded, exposing employees to caught-in hazards during operation of the feed mill. (b) During feed milling: On or prior to 12/04/22, the unload auger drive motor and belt assembly(s) at the "painted feed bin" were not guarded, exposing employees to caught-in hazards during unload auger operation.
