29 CFR  1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  Employees were engaged in activities related to tortilla manufacturing at a factory that is located at 10521 South 15th St., Grand Island, NE 68801, and were exposed to amputation hazards while conducting operations on machines.  An employee suffered an amputation and the employer had not developed energy control procedures.

Employees were engaged in activities related to tortilla manufacturing at a factory the is located at 10521 South 15th St., Grand Island, NE 68801, and were exposed to struck-by hazards in that the employees were not instructed in the recognition of the hazards associated with operation of powered industrial trucks, and the methods to mitigate those hazards.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Employees were engaged in activities related to tortilla manufacturing at a factory that is located at 10521 South 15th St., Grand Island, NE 68801, and were exposed to amputation hazards while operating a machine in that the employees were not instructed in the recognition of the hazards associated with cleaning a machine while it is operating, and the methods to mitigate those hazards.

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  An employee was engaged in tortilla manufacturing activities, and sustained an amputation of the employee's arm while cleaning an operating tortilla making machine at a facility that is located at 111 East 4th St., Grand Island, NE 68801, on November 20th, 2022, and was not reported to OSHA until December 7th, 2022.
