29 CFR  1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch  a) On or about December 6, 2022, at a worksite located 2401 Hawkins Point Road, Baltimore MD, throughout the facility; the work rest on the grinders were not properly adjusted to the wheel with gaps greater than one-eight inch exposing them to potential lacerations.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On or about December 6, 2022, at a worksite located 2401 Hawkins Point Road, Baltimore MD, throughout the facility; the tongue guard on the grinders were not adjusted to guard against particles if the wheel failed.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about December 6, 2022, at a worksite located 2401 Hawkins Point Road, Baltimore MD; Throughout the site, the employees were using electrical equipment such as but not limited to extension cords made by using extension cords and wall mounted multi-outlets which are designed to be placed mounted on a wall.  b) On December 6, 2022, at a worksite located 2401 Hawkins Point Road, Baltimore MD; Throughout the site, the employees were using electrical equipment such as but not limited to surge protectors outdoors which are designed for office use.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1910.305(a)(2)(x):  Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  a) On or about December 6, 2022, at a worksite located 2401 Hawkins Point Road, Baltimore MD; Throughout the site; employees had portable extension cords running through sharp metal holes exposing employees to an electrical shock hazard.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
