OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to fall hazards:   a. 1 Aspen Dr., #1., Randolph, NJ 07869  Employees engaged in merchandise inventory activities used an in-house manufactured work platform attached to equipment that included, but was not limited to, a Crown, 4500 Series Cascade Side Shifter, S/N: 9A150288, to be elevated on the forks, to adjust pallets on the racking system, count inventory, and add inventory tracking numbers to the products.  Employees were elevated on the work platform,  which had guardrails on only two sides,  was not secured to the forks to prevent slipping and/or tipping, and were not provided a personal fall arrest system to prevent falls when working elevated.  Violation occurred on or about 12/03/2022.
