1910.334(a)(2)(i)  Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):   The employer was failing to ensure that employees are not exposed to electrical shock hazards. This was most recently documented on December 6, 2022 at 4501 South 36th Street, Omaha, Nebraska in All American Meats' hamburger production area. Employees were working near exposed conductors of the 120V electrical wiring that powered the metal detector.
