29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about December 6, 2022, employees performed servicing and maintenance activities such as but not limited to clearing plugs on the Dryer 3 Full LTV Infeed. (a) The LOTO procedure did not include the rules and techniques to be used to control hazardous energy such as but not limited to gravitational, electrical and pneumatic. (b) The LOTO procedure did not include the means to enforce compliance.

29 CFR 1910.147(c)(6): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. The periodic inspection must: On or about December 6, 2022, employees performed servicing and/or maintenance on equipment including but not limited to Dryer No. 3 using lockout tagout procedures that had not been inspected within a year to identify deficiencies such as but not limited to the following: (a) No specific procedural steps with instructions (b) No purpose outlining the use of the procedure (c) No techniques to be utilized were listed (d) No specific steps for controlling stored energy

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 6, 2022, employees were exposed to machine hazards associated moving parts and in-going nip points while they performed servicing and maintenance to include but not limited to clearing plug ups or removing jams from Dryer No. 3. The machine was not shut down or turned off during this operation to remove the plug up; nor were the remaining applicable energy control procedures followed involving machine isolation [d(3)], LOTO device application [d(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)].
