29 CFR 1926.960(c)(1)(i): The employer did not establish minimum approach distances (MAD) no less than the distances computed by Table V-2 for ac systems. Location: Eversource substation, 129 Danbury Rd., Wilton, CT. On or about December 6, 2022, the employer did not ensure that employees who were required to work in close proximity to an energized electrical terminal board were protected against electric shock by establishing a minimum approach distance in accordance with Table V-2 for alternating current (AC) systems.
