29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about December 07, 2022, at a commercial construction site in Katy, TX employees were exposed to a fall hazard above 6 feet while performing metal framing work, while in an aerial lift without the use of fall protection.

29 CFR 1926.453(b)(2)(vii): Wheel chocks shall be installed before using an aerial lift on an incline, provided they can be safely installed. Brakes, outriggers, pads and/or wheel chocks were not used as required. On or about December 07, 2022, at a commercial construction site in Katy, TX, employer failed to ensure that aerial lifts were chocked on incline before use.
