29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets. On or about December 7, 2022, and at times prior thereto, an employee was cutting 2 by 4 pieces of wood without the use of a hard hat while working below another employee who had been installing joists, exposing the employee to the hazard of being struck-by falling debris, tools and materials.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about December 7, 2022, and at times prior thereto, employees used pneumatic nail guns to install joists and saws to cut wood at a residential home under new construction without wearing eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction 6 feet (1.8 m) or more above lower levels were not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented. On or about December 7, 2022, and at times prior thereto, an employee had been installing joists to a residential home under new construction without the use of a fall protection system, exposing the employees to a hazard of falling approximately eighteen feet to the ground.
