29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: a) On or About December 9, 2022 at the facility located at 2222 Cameron Street Alley 2 Lafayette La 70506, an employee was allowed to wear a Miller LPR-100 half mask respirator without being provided the basic advisory information required by Appendix D on non mandatory use of respirators.
