29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about December 7, 2022, two employees were working on the second-floor roof of a residential apartment building using pneumatic nail guns without using eye protection, exposing employees to the hazard of being struck by flying particles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about September 07, 2022, five employees were performing roofing operations on a residential apartment building undergoing a roofing remodel, up to approximately 18 feet high without fall protection, exposing the employees to fall hazards.
