The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).  c) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iv).  d) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures.The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).  e) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).  f) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022,and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures.
