29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  a)LLA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide medical evaluations to employees required to use the respirators as required in 1910.134(c)(1)(ii).  b) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide fit testing procedures for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iii).  c) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide procedures for proper use of respirators in routine and reasonably foreseeable emergency situations for tight-fitting respirators to employees required to use the respirators as required in 1910.134(c)(1)(iv).  d) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures.The employer did not provide procedures for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators of employees required to use the respirators as required in 1910.134(c)(1)(v).  e) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not provide training on putting on, removing, or the limitations of the respirators for employees required to use the respirators as required in 1910.134(c)(1)(viii).  f) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022,and at times prior, employees were exposed to health hazards when they conducted activities that required respiratory protection such as spraying lacquer without access to a written respiratory protection program that outlines those specific workplace procedures. The employer did not establish work-site specific procedures to regularly evaluate the effectiveness of the program as required in 1910.134(c)(1)(ix).

29 CFR  1910.134(g)(1)(i)(A):  Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  (a) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On or before December 7 ,2022 employees were exposed to potential chemical hazards while wearing a mandatory half-face respirator with facial hair that interfered with the seal. Employees wear respirators with long facial hair while spraying Amarium Pro PC Laq Satin.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical  (a) LA Woodworks Inc, 4476 Bents Drive, Windsor, CO 80550: On and before December 7, 2022, the employer failed to ensure secondary containers of acetone used in the fabrication shop were not labeled correctly. Squirt bottles were found at various locations around the shop without any identification regarding the chemical or information regarding the physical and health hazards of the chemicals.
