29 CFR  1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  (a) North side of commercial building; A worker using a Genie model S-45 extensible boom lift, serial no. S4506-11032, to egress the roof was standing outside of the basket while holding onto the guardrail as the basket was lowered from an approximate height of 17-feet above ground level.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a) Commercial roof structure, various locations; Workers installing steel roof panels from a low-slope roof were not protected from falls while working at approximate heights ranging between 17-feet to 29-feet, 3-inches above ground level.
