29 CFR 1926.1203(a):Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter through consideration and evaluation of the elements of that space, including testing as necessary: On or about December 5, 2022, at a construction site located at 940 N.E. 13th Street in Oklahoma City, OK, a competent person did not evaluate all confined spaces prior to allowing an employee to enter and work within the space, exposing the employee to fall hazards.
