29 CFR 1910.25(b)(3): The employer did not ensure that stairs have uniform riser heights and tread depths between landings.  a)  At Southern Grouts & Mortars Inc., located at 3303 State Road 574 W., Plant City, FL 33567:  On or about 12/08/2022 the employer exposed employees to fall injuries, in that the stairs used to access the stairway landing used as an operating platform of the Old Side Mixing Unit did not have uniform riser heights.

29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   a)  At Southern Grouts & Mortars Inc., located at 3303 State Road 574 W., Plant City, FL 33567:  On or about 12/08/2022 the employer exposed employees to fall hazards, in that at the Old Side Mixing Unit platform, which is 56 inches in height, or more than 4 feet above the next lower level, had two unprotected sides or edges that were not protected by a guardrail system.

29 CFR 1910.28(b)(7): The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of one of the systems listed in 29 CFR 1910.28(b)(7)(i)-(iv).  a)  At Southern Grouts & Mortars Inc., located at 3303 State Road 574 W., Plant City, FL 33567:  On or about 12/08/2022, on the upper deck of the Old Side Mixer, the employer exposed employees to fall injuries, in that an opening on a walking-working surface where the inside bottom edge was less than 39 inches above the walking-working surface and the outside bottom edge of the opening was more than 4 feet above the ground outside  was not protected from falling by use of a guardrail system, safety net system, travel restraint system or personal fall arrest system.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground may not be used for storage.    a)  At Southern Grouts & Mortars Inc., located at 3303 State Road 574 W., Plant City, FL 33567: On or about  12/08/2022, the employer exposed employees to electrocution hazard, in that:    a) Electrical panel "M" on the North wall of the production area was blocked with stored items , such as an industrial fan and cleaning equipment not allowing sufficient access and working space to permit ready and safe operation or maintenance. b) Electrical panel "I" on the South wall of the production area, near Scott I was blocked with stored items, such as pallets and containers of mixing ingredients,   not allowing sufficient access and working space to permit ready and safe operation or maintenance. c) Electrical panel "J" on the South wall of the production area, near Scott I was blocked with stored items, such as pallets and containers of mixing ingredients,   not allowing sufficient access and working space to permit ready and safe operation or maintenance. d )Electrical panel "K" on the South wall of the production area, near Scott I was blocked with stored items, such as pallets and containers of mixing ingredients,   not allowing sufficient access and working space to permit ready and safe operation or maintenance. e) Electrical panel "N" on the South wall of the production area, near Scott I was blocked with stored items  not allowing sufficient access and working space to permit ready and safe operation or maintenance.
