29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  3225 Ivy Terrace, Lot#13, San Antonio, TX: On or about December 07, 2022, and at times prior thereto workers were exposed to fall hazards while climbing an extension ladder whose side rails did not extend three feet above the roof top they were installing roof paneling to.
