29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each workshift for mildew, wear, damage, and other deterioration, and defective components are removed from service: On or about December 6, 2022, and at times prior thereto, an employee installing roofs onto trailers was in use of a fall arrest system that was not inspected for damage prior to initial use. This exposed the employee to a fall hazard.
