29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a. On or about December 5, 2022, at the above addressed jobsite, employees using a steel frame scaffold to perform stucco operations were not provided with an access ladder to upper and lower levels, thereby exposing the employees to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a. On or about December 5, 2022, at the above addressed jobsite, employees conducting stucco operations from a steel frame scaffold, approximately thirty-two (32) feet above the ground, were exposed to fall hazards due to the lack of a guardrail system, at the ends of the fame scaffold platform.
