29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on flat roofs with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, warning line system, or a safety monitoring system. (a) South side of roof - On or about December 9, 2022, the employer did not ensure the workers on the roof were protected from falling with the use of a fall protection system exposing them to fall hazards.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178. EACH 1910 STANDARD USED MUST INCLUDE "AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d)" at the end of the SAVE. (a) Throughout job site - On or about December 9, 2022, the employer did not ensure the all-terrain forklift operator had received the required training before operating the forklift exposing employees to struck by hazards.
