29 CFR  1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  On or about December 6, 2022,  Employees were exposed to exit routes that were blocked and obstructed with tools, materials, and equipment in buildings, including but not limited to, Building 5, Building 8 and Building 78.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.106(d)(6)(iv): Storage area(s) for flammable or combustible liquids were not kept free of weeds, debris and other combustible material.  (a)  On or about December 6, 2022, Rigging Shop,  Employees were exposed to a fire hazard where cardboard boxes and other combustibles were stored in a flammable cabinet with Isopropanol, a Category 2 flammable liquid.  (b)  On or about December 6, 2022, Rigging Shop,  Employees were exposed to a fire hazard where a cardboard box was stored with a Category 3 flammable liquid.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  U.S. Coast Guard, 2401 Hawkins Point Road, Baltimore, MD 21226:  (a) Throughout the Yard:  On or about December 6, 2022, Employees were required to wear tight-fitting respirators, PAPRs and air line respirators when performing work activities including, but not limited to, welding, painting, and grinding while the employer did not implement a site-specific respiratory protection program.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR  1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level:  (a)  On or about December 6, 2022, Building 11 Blast Building and throughout the yard, Employees were exposed to paint chips, with a lead content ranging from 0.0685% to 0.2134%, falling onto their work surfaces, tools, and equipment while the employer did not ensure initial exposure monitoring had been conducted.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
