South Hadley, MA: On or about December 2, 2022,  an employee was observed working on a residential structure engaged in window installation activities with unprotected sides and edges greater than 12 feet above the next lower level and he was not protected from falls by either guard rails, safety net systems or personal fall arrest systems.
