29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Scaffold frames were used as a means of access. Location: a) Mobile scaffold on the ground level of the jobsite: On or about December 5, 2022, an employee was erecting a sidewalk shed while standing on the first-tier platform of a one-tier-high mobile scaffold. There was no safe means of access provided. The employee had to climb the scaffold frame and crossbrace to get to the platform.

Location: a) Mobile scaffold on the ground level of the jobsite: On or about December 5, 2022, an employee was erecting a sidewalk shed while standing on the first-tier platform of a one-tier-high mobile scaffold. The scaffold wheels were not locked to prevent movement of the scaffold.
