On or about December 12, 2022 and times prior there to, portable fire extinguishers (PFE) that were mounted in the facility were not clearly identified with some sort of physical sign or label at the following instances: a) Two PFE adjacent to Room 162 and 169 in B Hall; b) One PFE adjacent to Room 116 in M Hall; c) One PFE in Break Room in C Hall; d) One PFE in Laundry Department in E Hall; e) One PFE adjacent to Room 260 in E Hall; f) One PFE adjacent to Room 276 in E Hall; g) One PFE in Laundry in F Hall; h) One PFE adjacent to Room 288 in Hall F; i) One PFE adjacent to Room 304 in Hall F; j) One PFE at the Nurses' Station in M-Hall
