29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Axlehire, located at 5039 Comly St., Philadelphia PA 19129: On or about December 13, 2022, the employer did not ensure that each powered industrial truck operator received training and evaluation prior to operating powered industrial trucks, thereby exposing employees to struck-by hazards. Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.
