Employees were exposed to hazards associated with fire and combustion byproducts in that resorcinol resin material was accumulating on surfaces and piping above kettle 3.

Employees working in the building #40 control room were exposed to significant overpressure explosion and deflagration hazards from the process area in that the control room was not equipped with appropriate blast resistant construction or other protective measures.
