NOTE: Abatement certification AND documentation are required for this item.  HMH Construction LLC was previously cited for a violation of this occupational safety and health standard 1926.21(b)(2), which was contained in OSHA inspection number 1455786, Citation 1, Item 1 a and was affirmed as a final order on March 23rd, 2020, with respect to a workplace located at 3500 West Franklin Road, Meridian, ID 83642.

29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a) On December 13, 2022; employees working to break up soil with a jackhammer  in a trench without cave-in protection used a ramp located approximately 20 feet away from their location of work. The trench was composed of Type B soil and measured approximately 6.4 feet deep, 12.5 feet wide, and 25 feet long.   NOTE - Abatement certification AND documentation are required for this item.  HMH Construction LLC was previously cited for a violation of this occupational safety and health standard 1926.651(c)(2), which was contained in OSHA inspection number 1455786, Citation 1, Item 3 and was affirmed as a final order on March 23rd, 2020, with respect to a workplace located at 3500 West Franklin Road, Meridian, ID 83642.

29 CFR  1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.   a) On December 13, 2022; the competent person did not remove employees who were working in a  6.4 foot deep, 12 foot wide, and 37 foot long trench without cave-in protection. The trench had 72 degree walls, Type B soil and multiple sources of vibration.  NOTE: Abatement certification and documentation are required for this item.  HMH Construction LLC was previously cited for a violation of this occupational safety and health standard 1926.651(k)(1), which was contained in OSHA inspection number 1455786, Citation 1, Item 1b, and was affirmed as a final order on March 23rd, 2020, with respect to a workplace located at 3500 West Franklin Road, Meridian, ID 83642.
