Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) Yard area: On or about December 13, 2022, and times prior thereto; the employer exposed employees to crush-by and struck-by hazards in that employees were allowed to operate forklifts with attachments that had not been approved for use by the manufacturer of the powered industrial truck to move slabs of stone.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) Front Warehouse/Production Area: On or about December 13, 2022, and at times prior thereto; the employer exposed employees to electrical/fire hazards in that the employer extended extension cables by connecting additional extension cables in series in order to provide power to multiple lights attached to the ceiling using one outlet, which was against manufacturer specifications for use.
