Spruce St. St. Marys OH 45885, did not ensure the side rails of one portable Werner brand aluminum extension ladder was extended at least three feet above the landing surface. Five employees were exposed to falls from approximately 10 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single family home.
