On or about December 13, 2022 the employer Antonio Bolanos Pablo DBA Pablos Construction at jobsite located at 904 N. Spruce St. St. Marys OH 45885, did not ensure that each affected employee used appropriate eye or face protection when preforming roofing work on a existing residential structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns.

Spruce St. St. Marys OH 45885, did not ensure employees were protected from falls while conducting residential roofing construction activities on the second (roof) level of a residential single family home, while exposed to a falls of approximately 18 feet and 10 feet or greater to the ground below.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 13, 2022 the employer Antonio Bolanos Pablo DBA Pablos Construction at jobsite located at 904 N. Spruce St. St. Marys OH 45885, did not ensure the side rails of one portable Werner brand aluminum extension ladder was extended at least three feet above the landing surface. Five employees were exposed to falls from approximately 10 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform residential roofing activities on a single family home.
