29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) On or about January 4, 2023, employees conducted lockout/tagout on the Tripe Machine to conduct sanitization work. The energy control procedure did not identify and include procedures for locking-out the pneumatics on the equipment, exposing employees to a caught-in and crushing hazard.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: (a) On or about January 4, 2023, employees of a contractor conducted sanitation work on equipment in the facility which required the employees to lockout/tagout equipment, and the host company and contractor did not share and/or communicate their program and procedures.
