29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. At a new residential construction site located at, or near 7028 Deer Lodge Circler, Jacksonville, Florida: On or about December 14, 2022, and at times prior, the employer exposed employees to struck-by hazards in that eye protection was not provided to employees, while installing the shingles on top of the roof.  The Brayden Roofing Specialists, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard which 1926.102(a)(1) was contained in OSHA inspection number 1594838, citation number 1, item number 2 and was affirmed as a final order on 07/06/2022, with respect to a workplace located at 309 Old Plantation Drive, Saint Augustine, FL 32086.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. At a new residential construction site located at, or near 7028 Deer Lodge Circler, Jacksonville, Florida: On or about December 14, 2022, and at times prior, the employer exposed employees to fall hazards, in that fall protection was not utilized by employees, while installing the shingles on top of the roof.  The Brayden Roofing Specialists, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 1594838 citation number 1 item number 3 and was affirmed as a final order on 07/06/2022, with respect to a workplace located at 309 Old Plantation Drive, Saint Augustine, FL 32086.
