29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.  a. Front Roof area located at 309 Old Plantation Drive, Saint Augustine, Florida 32086: On or about May 6, 2022, the employer exposed an employee to head injury hazards, in that a hard hat was not utilized by the employee who was working underneath other employees on the lower portion of the roof to install trim.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. Roof area at a worksite located at 309 Old Plantation Drive in Saint Augustine, Florida 32086: On or about May 6, 2022, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail gun to nail down roof shingles, install weather proofing and install trim work.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 9:12 pitched roof, at a worksite located at 309 Old Plantation Drive, in Saint Augustine, Florida 32086: On or about May 6, 2022, the employer exposed employees to fall hazards of 9 - 30 feet to the ground below while engaged in roofing activities in that the employer failed to ensure fall protection was utilized.
