29 CFR  1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:   a. On residential roof, at a temporary worksite located at 6467 Heidi Road, in Jacksonville, Florida: On or about August 9, 2023, the employer exposed employees to fall hazards, in that the ladder rails did not extend 3 feet above the upper landing surface of the roof at the eave.

Employees performing roofing work at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against fall as required by the standard, as demonstrated on August 9, 2023, when three employees performing roofing work on a 3:12 pitch roof of a house located at 6467 Heidi Road, in Jacksonville, Florida, were exposed to the hazard of a 10.3 foot fall to the ground in that they did not use a guardrail system, safety net system, personal fall arrest system or any alternative fall protection system.
