The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about December 6, 2022, the employer did not ensure employees performing service and maintenance activities on the Mori Seiki NH4000 DCG applied energy controls by; turning off or shutting down the machine[per 1910.147(d)(2)], isolating the electrical and hydraulic energy sources [(d)(3)], affixing lockout device(s) to the energy isolation devices [(d)(4)(i)], and verifying de-energization and isolation had been accomplished [(d)(6)], prior to unjamming product inside the machine.
