29 CFR 1904.29(b)(1): The Log of all Work-Related Injuries and Illnesses (OSHA Form 300) and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or equivalent forms were not recorded in detail required by the respective forms: a.) Scott's Splash Lagoon, Inc., on or about December 15th, 2022 and times prior thereto: The OSHA Form 300 for calendar year 2022 did not provide specific detail about the object/substance that directly injured or made the employee ill, in Column F. Case number 4, did not provide specific detail about the object/substance that directly injured or made the employee ill. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
