29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial vehicle operator's performance was not being conducted at least once every three years: On or about February 1, 2023, the employer did not ensure powered industrial vehicle operators were evaluated for operator performance every three years. Employees operate powered industrial vehicles to move product from the production area to different storage and shipping locations throughout the facility. Employees were exposed to struck-by injuries, such as being ran over by the powered industrial vehicles, material falling from the powered industrial vehicles, and injuries associated with uneven docks/loading ramps due to the lack of refresher training.
