On or about December 15, 2022, and at times prior thereto, at the residential construction site, the employer did not ensure that employees working on the first level of the scaffold had a safe means of access/egress, exposing employees to fall hazards at approximately twelve (12) feet above the ground when using the cross braces/end rails.
