29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On December 15th, 2022 employees were using a section of extension ladder which did not extend 3 feet above the upper landing being accessed.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On December 15, 2022  an employee working in a trench was not wearing protective helmet where there was a possible danger of head injury from falling objects.  TJW Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in OSHA inspection number 1550683 , citation 1, item 1, and was affirmed final order on October 22, 2021, with respect to a workplace located at 3751 N. Lakewood Ave, Chicago, IL 60613  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:  a) On December 15, 2022,  employees working in an excavation up to approximately 6.3 feet in depth were exposed to cave-in hazards.  TJW Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1550683, citation 1, item 3, and was affirmed final order on October 22, 2021, with respect to a workplace located at 3751 N. Lakewood Ave, Chicago, IL 60613.  No abatement certification or documentation is required for this item.
