29 CFR  1910.146(k)(3)(i):  Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant.   a) On or about July 12, 2022 through July 15, 2022, NAVFAC Midlant dba PWD Great Lakes did not ensure that employees utilize chest or full body harness with a retrieval line attached when they entered steam vault ST1.5 at the Captain James A. Lovell Federal Health Care Center in order to perform a welded repair to a six inch, high pressure steam line.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
