(a) South side of house exterior - On or about December 16, 2022, the employer had his employees build and use the wooden scaffolding to frame the house and the scaffolding planks are not 18 inches wide exposing them to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) South side of exterior of house - On or about December 16, 2022, the employer had his employees build and use the wooden scaffolding to frame the house and did not provide fall protection for employees exposing them to fall hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  (a) Front of house - On or around December 16, 2022, the employer instructed his employees to use the aerial lift and did not provide a safety harness for them to use exposing them to fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Top of house - On or about December 16, 2022, the employer instructed his employees to apply the roof sheeting to the two-story house without providing fall protection exposing employees to fall hazards.
