On or about December 16, 2022, at an establishment located at N4040 County Road C in Chilton, Wisconsin; The employer did not provide a program and train employees in safety procedures to be followed when servicing single-piece and multi-piece rim wheels.   Training program elements are defined in paragraphs (c)(1)(i) through (c)(1)(iii) of this section. Employees shall be trained in the safe operating procedures described in paragraphs (f) and (g) of this section for multi-piece and single-piece rim wheels.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  On or about December 16, 2022, at an establishment located at N4040 County Road C in Chilton, Wisconsin; the employer did not obtain written approval from the forklift manufacturer prior to the installation of an attachment on four forklifts. The right forks were modified with a hole drilled into the tip to secure the attachment with a pin. Capacity plates supplied on the vehicles did not include lifting capacity limitations nor safe operating instructions for the attachment.
