29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about December 14, 2023 at the 61 Chapel Hill Re-Roofing Project,  an employee was on the roof deck installing granulated ice and water shield and slipped on the material and fell off the roof deck to the ground below.

29 CFR  1926.1053(b)(21):Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:  On or about December 14, 2023 at the 61 Chapel Hill Re-Roofing Project,  employees were holding and carrying rolls of Ice and Water Shield on one shoulder and were leapfrogging with their other hand from rung to rung while climbing the ladder to the roof deck.
